Yan Chai Hospital - Family Love Connects Project
Service operation period: 2021-2023
Introduction:
Family Love Connects Project was funded by the Bank of China. Service team includes different healthcare professionals, such as; Physiotherapist, Occupational Therapist, Nurse, Social Worker, Dietitian, and Fitness trainer. These professionals will support carers in term of caring skills and knowledge. Carers can also join the project’s activities and to earn service points. These service points can be used to redeem different social services to support their caring and relieve their stress. These service points redemptions can be escort service, day care center service, etc.
Target Service User:
- Carer of YCH community elderly service units; or,
- Carer from the community (aged 18 or above, who are taking care of their elderly family members); or,
- Elderly who lived in the community (aged 60 or above, their carer are the project participants)
Facility:
1 Main Office and 3 Satellite Centres. Fitness and rehabilitation equipment provided in the Satellite Centres.
