Yan Chai Hospital - Family Love Connects Project

Introduction:
Family Love Connects Project (FLCP) is funded by the Bank of China (Hong Kong) Limited. Our multi-disciplinary team is composed of different healthcare professionals, including Physiotherapist, Occupational Therapist, Nurse, Social Worker, Dietitian, and Fitness trainer. FLCP strives to support and relieve carers’ stress by equipping them with caring skills and health-related knowledge. Carers can partake in FLCP activities to earn service points. Using the mobile application “Blessing Ones with Care Saving Account”, these points can be used to redeem community services, e.g. escort services and daycare center services. In addition, rehabilitation aids, health monitoring or assistive devices are also available for exchange.
Target Service User:
- Carer of YCH community elderly service units; or,
- Carer from the community (aged 18 or above, who are taking care of their elderly family members); or,
- Elderly who lived in the community (aged 60 or above, their carer are the project participants)
Facility:
1 Main Office and 3 Satellite Centres. Fitness and rehabilitation equipment provided in the Satellite Centres.
